• You have already submitted your proof of vaccination to Halton Region.
• Your record has already been documented by another health unit.

Before completing the form, please carefully review the following:

• Do not submit records of COVID-19 vaccinations received in Ontario. This form is for COVID-19 vaccination records received outside of Ontario only. After assessment, valid vaccine records will be added to the Provincial COVID vaccination system.
• Your COVID-19 vaccination records must be in English or French, or you must have a certified translation into English or French.
• This form is for Halton Region residents only. If you live outside of Halton Region, contact your Public Health Unit. Find your Public Health Unit.
• This form is not for travel documentation purposes. If you require to submit your COVID-19 vaccination records for travel purposes, please visit ArriveCAN for more information.
• We are no longer able to accept handwritten proof of vaccination documents on their own. Handwritten proof of vaccination documents need to be accompanied by a scannable QR code or additional electronic receipt issued by the clinic/pharmacy/health authority. To avoid further delay, first see if you are able to obtain a SMART Health QR code.
  • If your jurisdiction is on the SMART health card list, contact the clinic or health authority (or visit their website) who provided your vaccination and request your SMART Health Card QR code.
  • If your jurisdiction is not on the list, please contact the clinic or health authority who provided your vaccination and request additional proof of vaccination (for example, scannable QR code, prescription receipt from the pharmacy, or an electronic document from the clinic or region where your record is from).
